MyChart
MyChart is a secure, online patient portal that makes it easy for you to access your health information from anywhere, at any time and at no cost. Using MyChart can help you to manage and engage in your health care.
With MyChart, you can see your –
- after-visit summaries
- upcoming appointments
- test results
- progress notes
- discharge summaries
- medical imaging reports
- patient education materials
– from visits to KDH and any of our healthcare partners listed in the next section.
If you have notifications turned on, you will get an email or notification from the MyChart app when tests, labs and reports are ready and available to view on your MyChart. Your results will be reviewed with you at your next appointment. If your results are more medically urgent, someone from your healthcare team will call you sooner to discuss.

Your Health Record
Your Personal Health Information from KDH and our healthcare partners listed above can be viewed in your MyChart account.
You will see a list of your health issues, allergies, and medications, most lab and imaging results, upcoming and past outpatient clinic appointments, along with summaries from your visits, and visit summaries from the Emergency Department and inpatient units. You can add health measurements such as blood pressure and weight when ordered by your care provider.
MyChart will show your medical information going back as far as 2016 in most cases. If you have had a hospital visit within the last five years and it is not visible in MyChart, contact mychartkdh@kdh.on.ca and our team will investigate. To access your entire health record, you will need to visit Health Records and request a complete copy of your record.
To see your test results, click on “Health” and select “Test Results.” On the “Test Results” page, click anywhere within the row to display the results for that test. Your lab results, diagnostic imaging and pathology results will be available after the completion of the reporting on the studies. Please note that some results will not be available in MyChart.
Note: You may be able to see your results before your healthcare team as they are reviewing results for many patients. Your results and any questions you may have about them will be reviewed with you at your next appointment. If your results are more medically urgent, someone from your healthcare team will call you sooner to discuss.
To view clinical notes, log on to MyChart and click on the “Health” tab and select “Appointments and Visits.” If applicable, clinical notes will be available under “After Visit Summary” or “Notes” tab. In clinical notes you can view after visit summaries, operative notes, and discharge summaries.

Yes, MyChart for iOS and MyChart for Android are portable versions of MyChart that you can use to manage your health information on the go! The mobile apps contain many of the same features as the MyChart website. To install the MyChart app, go to the App Store or Google Play Store and search for “MyChart.”
Your healthcare provider reviews and updates your health information in the electronic health system during and after each visit. Ask your healthcare provider to correct any incorrect information at your next clinic visit. Patients and proxies can add and update information about allergies and medications through MyChart. Once this information is added, your healthcare provider will discuss it during your next visit.
Yes, you can. This is called a proxy access. There is a procedure in place if you would like access to a patient’s MyChart or give someone access to your MyChart. Please ask about this feature during your next registration or visit our Health Records Department to complete a Proxy Consent Form.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password and the account cannot be accessed without that password.
Play your part in protecting your personal health information by:
- Giving accurate information when you sign up (for example, your email address, your name and your date of birth)
- Keeping all private information secure: your MyChart activation code, medical record number (MRN), username, password, and the security question you chose to recover your password if needed
- Setting a strong password. Your password should be at least eight characters and include one letter and one number. Use both upper and lower case letters with numbers and symbols to make your password stronger
- Protecting your device (computer or mobile device) with a password
- Using a password-protected screen lock with a timeout on your device
- Installing up-to-date personal firewall software, anti-malware, and anti-spyware protection
- Installing the latest updates to your operating system (Windows, iOS, etc.)
- Not logging into MyChart from a shared or public computer, where you cannot be sure of the device security
- Never responding to emails that ask you to change your MyChart username and password. We will never ask you to change your MyChart username or password by email.
On the MyChart login page, click ‘Forgot username’ or ‘Forgot password’ below the green Sign In button. Enter the required information on the screen that comes up, including your date of birth and health card number or medical record number (MRN). The next screen will explain the process to regain access. For a forgotten username, you will be sent a code by email that you will have to enter. Once this is done, you will be logged in. For a forgotten password, you will be sent an email to reset your password. If you continue to have trouble logging in, you can contact the support team at the hospital where you typically receive care.
For your security, your activation code expires after 14 days. Also, it is no longer valid after the first time you use it.
If your keyboard remains idle for more than 15 minutes, you will automatically be logged out. This helps protect your privacy and the security of your information. Log out of your MyChart session if you ever need to leave your computer, even for a short time.
You only receive email alerts when new information (like lab results, messages, and appointment reminders) appears in MyChart. If you are not getting alerts –
- Check your Junk Mail folder for email alerts. If you see messages there, change the settings on your email filter to allow these messages.
- Check your notification preferences in MyChart. Click on “Settings” and select “Notifications” to make sure your correct email address is entered and that you are signed up for the alerts you want to receive.
Some test results are only given in person or over the phone. These kinds of tests results are not normally available on MyChart. If you feel that your test results should be available, but you don’t see them, please contact your health care team.

“I wanted to tell you how truly grateful I am for the Clinics at KDH. Almost a year of injections for pain in Dr. Jamal’s clinic have been quite successful and after last Friday's appointment, I am now on "remit" until November. Through Dr. Jamal I was referred to Dr. Fox, who diagnosed Carpal Tunnel Syndrome in my right wrist, for which I had surgery with Dr. Chan – all through your Clinic system. Everyone in the Clinics has been so helpful and kind throughout this: Kerry and Chrissie at the door and June in Bookings. They have made my visits easier and contributed to my recovery. I feel they are friends, as well as the doctors. KDH is a great hospital with wonderful people. Carry on!”
